Employment Application

I understand that R-V Industries, Inc follows the usual practice of requiring new employees to sign an employment agreement at the time of employment, which also includes inventions and patents. I understand that this application is current for only 60 days. If I have not been contacted within 60 days, and still wish to be considered for employment, I must fill out a new application. In connection with my application for employment, I understand that investigative background reports will be made on myself, including consumer, criminal, driving, medical, and other reports. These reports will include information about my character, work habits, performance, and experience. Furthermore, I authorize the company to request information from various federal, state, and other agencies which maintain records concerning my past activities relating to my driving, criminal, medical, civil, and other experiences. I understand that my employment with R-V Industries, Inc. is contingent upon my successfully undergoing a pre-employment medical examination, including a drug screen. Furthermore, I undestand that if I am employed by the company, I may be required to submit medical examinations and/or test conducted by a license person, selected by the company, at their expense. I give authorization to any medical facility and to any records or information that results. I further authorize the release of my medical records and information that may be relevant to the investigation of any claim against the company or its insurance carriers - including any claim I may have for workers' compensation. I hereby acknowledge, understand, and agree that if I am hired by R-V Industries, Inc, my employment is not guaranteed and no representative of the company has any authority to enter into any oral or written agreements or statements contrary to the foregoing.
